2. Application Form. Please complete the application form and either deliver or mail it to Nick Aysseh at 253 Ruane Street, Fairfield, CT 06824 along with a check for $205 made payable to "Cub Scouts Pack 99". The $205 covers sign up fees, insurance and activities for the full year.
